Trillium Background

Trillium Community Health Plan is a Coordinated Care Organization (CCO) and recognized leader for innovative approaches and outstanding coordination of community-based healthcare. Helping create a healthier community is the core of Trillium’s mission. We partner with an extensive group of physical, behavioral and oral healthcare providers on behalf of Oregon Health Plan members in Lane and Western Douglas County.

Trillium is committed to its members and community. Since 2012, Trillium has invested more than $13 million in Lane County to open four new clinics to ensure all members have access to primary care, to expand prevention services targeted at reducing tobacco use and obesity, and to further integrate and enhance community behavioral health services – including addressing the holistic needs of our most vulnerable such as housing, education, and access to healthy food.

It all started in 1977 when local Lane County doctors came together to form the Lane Individual Physicians Association (LIPA). In 1996, as commercial insurers were backing out in Lane County, LIPA funded the creation of a plan to locally manage the Oregon Health Plan and was awarded by the state in 1999. In 2012, Trillium Community Health Plan was one of the first Coordinated Care Organizations (CCOs) approved by the state to transform care for Oregon Health Plan members.

From early in its development, Trillium collaborated with Lane County and formed a unique public-private partnership. The partnership has become a model for integration of behavioral health and primary care integration as well as expanding the reach of prevention efforts.

As a key focus area, Trillium sets aside $1.33 per Oregon Health Plan member per month to fund programs that prevent chronic disease and help Trillium members live healthier lives. Instead of just treating our members when they get sick, we work to keep them healthy and thriving. Trillium has invested more than $13 million in the community to expand access to care, enhance prevention programs targeting tobacco, obesity and behavioral health and fund grants that improve delivery of care through innovation.

We are proud of our 20+ years working on behalf of Oregon Health Plan members. Whether the need is for medical care, food, housing or transportation Trillium strives to meet the needs of each individual. We are more than a community-based organization serving members- we're part of the community.

Trillium Community Health Plan provides Oregon Health Plan (Medicaid) benefits for residents of Lane County, parts of Linn County and the Reedsport community of Douglas County. Starting 9/1/2020, members in Washington, Multnomah and Clackamas counties can choose Trillium as their CCO.
