Trillium NICU/Pediatrics Care Management

Trillium Community Health Plan has Nurse Care Managers on our Neonatal Intensive Care Unit (NICU) and Pediatrics team. They are specially trained to help parents, guardians, and resource parents meet the physical health needs of infants and children. This includes infants and children who are currently in the NICU. It also includes those who have recently been discharged from the NICU. To connect with us, call Trillium Member Services at 877-600-5472. Ask for Care Management.

The links below connect you to federal, state, local hospital, and non-profit programs. These programs can help support you and your child during your healthcare journey.

  • CaCoon: CaCoon is a public health nurse home visiting program. This program is available through OHSU. It helps families coordinate care for their children. 
  • NICH: NICH stands for “Novel Interventions in Children’s Healthcare.” This program is run by OHSU in partnership with Trillium’s Complex Care Management Team. The program provides an interventionist 24 hours a day, 7 days a week. This interventionist works with a Trillium Care Manager to support children with complex or long-term health needs.
  • Early Intervention / Early Childhood Special Education:
    This Oregon Department of Education program offers early intervention and special education for infants, toddlers, and preschoolers. This program helps support the whole family.
  • Bright Futures: The Maternal and Child Health Bureau manages this program. It outlines national standards for well-child checkups. This helps to improve preventive and primary care for children.
  • Parenting Now: This website is run by a non-profit organization that brings parents together for mutual support and learning. Groups meet for 10 or 12 weekly sessions with families who have children of the same age. A professional Parenting Educator facilitates each group. 
    Help Me Grow Oregon: This program offers a free, confidential family support line through the Swindells Resource Center at Providence Children’s Health. It connects families to community services starting in pregnancy and beyond.

Links for Adolescents

The links below offer national, state, and nonprofit resources for teens and young adults. These programs support health, mental well‑being, and the transition to adult healthcare. Many offer confidential peer support.

  • Got Transition: This program is from The National Alliance to Advance Adolescent Health, which is a national resource center on health care transition (HCT). This is a helpful resource as youth move towards adult healthcare.
  • NAMI Oregon: This website is run by the National Alliance on Mental Illness (NAMI). They offer a free Resource Helpline, in-person help, groups, classes, and more. 
  • Youth Health: This Oregon Health Authority webpage shares youth health programs. These include school-based health centers, school nursing, school health programs, and resources for youth sexual health.
  • Oregon Family Support Network: This website is run by the Oregon Family Support Network. They provide family peer support, training, advocacy, and more. 
  • Youth Era: This website is run by a non-profit organization in Oregon. They provide youth peer support for ages 12 – 25. Programs focus on mental health, empowerment, and learning how to navigate health and social systems. They have safe space drop-in centers, virtual support via social media, and more.
