Lane County Community Outreach

Live Healthy Lane is a community-based effort to improve the health and well-being of those who live, learn, work, and play in the Lane County Region. Live Healthy Lane unites community partners, local public health organizations, CCOs within the service region, and healthcare providers to reduce health disparities, promote health equity, and improve overall population health. Together we work on the Community Health Assessment (CHA), which explores and identifies health issues in Lane County.

The CHA creates an opportunity to improve the health of our community by informing the Regional Community Health Improvement Plan (CHP) (PDF).

The CHP is an action-oriented plan with goals to:

  1. increase economic and social opportunities
  2. increase health behaviors to improve health and well-being
  3. strengthen cross-sector collaborations

Tri-County Community Outreach

Trillium collaborates with local community partners to develop a Community Health Needs Assessment (CHNA) so we can best improve the lives of those who live, work, and play in the Tri-County area. 

Working closely with community partners, local public health organizations, CCOs within the service region, and healthcare providers, we identify specific strategies for improving health equity in the Tri-County communities. 

To develop our plan and ensure we meet the needs of minority and diverse Oregon Health Plan/Medicaid members, we:

  • Assess community need through input from local stakeholders
  • Jointly establish community health and equity priorities
  • Identify shared solutions
  • Fund training and initiatives for providers and community members
  • Track outcomes for continuous quality improvement
