Trillium Recognizes Medicaid Providers with CLAS Scores in Top 10 Percent
Every year, Trillium’s Medicaid providers have the opportunity to take the Provider Health Equity & Technology (PHET) Survey. We appreciate your feedback and use your insights to support culturally competent care, inform our community health equity initiatives, and develop provider resources to help improve access to care and health outcomes.
The Culturally and Linguistically Appropriate Services (CLAS) section of the survey evaluates an organization’s ability to advance health equity and increase access to care through meeting the cultural and language needs of our diverse Medicaid population.
Special thanks to our Trillium Medicaid providers who earned CLAS scores in the top 10 percent on the 2022 PHET Survey. View the list of providers with top 10 percent CLAS scores (PDF).
We appreciate our providers’ ongoing commitment to providing culturally responsive care for our Trillium members. If you have questions or would like more information about your 2022 PHET Survey results, please contact your Provider Relations Representative or call Provider Services at 1-877-600-5472.
And if you have not yet completed the 2023 PHET Survey, you can access the PHET survey now. Trillium contracted providers who complete this survey may be eligible to earn a bonus. Your responses are due by December 31, 2023.

Traditional Health Workers Available to Support Trillium Medicaid Members
Traditional Health Workers (THWs) are available to help Trillium members reach their health and wellness goals, understand their benefits, and connect to culturally responsive care. THWs may also provide support with finding resources for food, housing, rides, and more.
There is no cost to Trillium members for working with THWs. If you have a patient who might benefit from THW support, ask them to call Trillium Member Services at 1-877-600-5472 (TTY: 711) or email us at THW@Trilliumchp.com.

Reminder: Lane County Trillium Medicaid Behavioral Health Provider Availability Survey
In the Lane County CCO region, Trillium Behavioral Health has transitioned from monthly phone calls to an online survey to assess provider availability and your program openings. This will streamline the process for you as well as ensure we are able to provide members with the most accurate provider information. You should expect to receive the survey link via email on the first Monday of every month.
As always, we deeply appreciate the valuable work that you are doing for our Trillium members and our community at large. At the top of each survey, you will find contact information for Emma St. Clair and Erinn Stevens. Please reach out to both of them directly if you have any questions or concerns. Medicare Annual Enrollment Period for Wellcare By Health Net & Wellcare By Trillium Advantage
Medicare Annual Enrollment Period is here! You may receive calls from patients checking on which Medicare plans you accept.
Several years ago, we combined multiple brands under the Wellcare name to offer a better range of plans that provide members with affordable access to doctors, nurses, and specialists.
Health Net and Trillium Advantage are now Wellcare. As always, we are committed to working with you to ensure your patients receive the best care.

Trillium Community Health Plan is proud to be a member of ALL IN Lane County, the first multi-agency, county-wide effort aimed at reducing houselessness with an emergency focus. ALL IN has created a plan with three main focus areas: homelessness prevention, emergency shelter, and rehousing.
