Trillium is committed to partnering with you to support culturally competent care for our members. The Provider Network Assessment will help us determine how well we are meeting this goal and improve our ability to direct our members to you for appropriate care.
 
Please complete this assessment at your earliest convenience. Your feedback will provide vital practice information we are required to obtain by the state, as well as help us better support you. Your answers to questions related to Culturally and Linguistically Appropriate Services (CLAS) Standards and Language Access will be scored to give you an initial “CLAS Score.” If you need assistance in meeting the minimum requirement based on CLAS Standards, we will contact you directly to assist with training and resources.
