Coordinated Care Organization Changes for Lane County

What's Happening

  • PacificSource Community Solutions (“PacificSource”) will no longer be a coordinated care organization (CCO) in Lane County in 2026.
  • PacificSource will serve as a CCO in the region through January 31, 2026. 
  • Trillium Community Health Plan (“Trillium”) will take on coverage for most PacificSource members in Lane County beginning February 1, 2026. 
  • Trillium is working closely with PacificSource and the Oregon Health Authority (OHA) to ensure a smooth transition, making sure members have coverage and access to care without interruption.

What This Means for Current PacificSource CCO Members

  • You will not lose your healthcare.
  • There are no immediate changes to your benefits or providers. 
  • You should continue using your benefits and accessing care as usual.
  • During the transition, you will be automatically moved to Trillium unless you are American Indian/Alaska Native (AI/AN).
    • If you are an OHP member who is AI/AN and enrolled with PacificSource, you can choose to enroll in either Trillium or Open Card.
    • You will automatically move to Open Card if you do not choose Trillium by February 1.  

What This Means for Current Trillium CCO Members

  • There will be no change for current Trillium CCO members. Trillium members will continue to have access to high-quality healthcare and services.

Transition of Care: What You Need to Know

  • When you move to Trillium, you may still get care that was already approved. This is called a Transition of Care. It helps make sure your treatment continues without a break.
  • Trillium will receive approved authorizations from PacificSource. 
  • You can keep getting approved services for a limited time. This is usually 30, 60, or 90 days depending on your healthcare needs and the type of care. 

About Your Provider

  • New members on Trillium's plan will receive a member packet. The packet will include instructions on how to determine whether your provider is in-network or out-of-network. New members will receive these packets before February 1, 2026.
  • Trillium is always working to add new providers to our network. It is a good idea to keep checking with us if your provider isn't listed yet. We listen to your feedback and work to make sure we are meeting the needs of our members and the communities we serve.
  • Between March 2 and April 2, members who do not have an in-network provider will automatically be assigned one. Members may also contact us directly at 1-877-600-5472 for help. You can visit our online provider search tool to check if a provider is in network.

After the Transition of Care Period

Once the transition of care period ends, whether you can keep seeing your current providers depends on if they are part of Trillium's network. To find out:

Transition of Care Period

  • New Trillium members can keep getting approved services from their previous providers during the Transition of Care period. This is usually 30, 60, or 90 days depending on healthcare needs and the type of care. 
  • After the Transition of Care period ends, non-contracted providers may continue to provide covered services to Trillium members with an approved authorization. To learn more, visit the Trillium Prior Authorization webpage for providers.
