Trillium Medicaid Providers: Complete the Required Provider Health Equity & Technology Census by December 31

Trillium is committed to partnering with you to support culturally and linguistically appropriate care for our members. Every year, the Oregon Health Authority (OHA) requires Trillium to collect data from providers about health equity, language services, disability access and telehealth.

Trillium and OHA use this information to:

  • Identify areas where we need to expand our network
  • Determine electronic health record (EHR) needs
  • Inform community health equity initiatives
  • Develop provider resources to help improve access to care and health outcomes

Please submit your data by 5 p.m. on Tuesday, December 31, 2024 using Trillium’s Provider Health Equity & Technology Census Form. The form takes approximately 30 minutes to complete.

Note: If you would like to gather the information before submitting it in the census form, please view the Provider Health Equity and Technology Census in PDF format.

If you have any questions, reach out to your Provider Engagement Representative or call Provider Services at 1-877-600-5472.

Harm Reduction & Bridges to Care Clinic: Substance Use Disorder Treatment

The Harm Reduction & Bridges to Care clinic (HRBR) is a collaboration between OHSU and the Oregon Health Authority. This virtual clinic provides evidence-based, low barrier, on-demand access to medications for addiction treatment to people who are interested in cutting back or stopping their drug or alcohol use.

The HRBR team consists of physicians, advance practice providers, a social worker, care transitions coordinators, patient access specialists, and certified recovery mentors/peer support specialists. The clinic accepts any insurance and no insurance. Patients without insurance will be referred to OHSU Patient Financial Assistance to screen for insurance and financial assistance eligibility.

To engage in treatment, members just need access to a tablet or smart phone and be physically located in Oregon.

For more information, please visit the HRBR website or contact your assigned health plan Provider Engagement Representative.
