Trillium Community Health Plan is hosting virtual office hours to help answer your questions about the Community Capacity Building Funding (CCBF) grant for the new Health-Related Social Needs (HRSN) benefits.
The 2024 Community Capacity Building Funding (CCBF) grant application window opened 3/1/24 and closes 5/31/24.
