Critical Updates from Trillium's Recent CCBF Office Hours
Thank you for participating in Trillium Community Health Plan’s recent office hours. Your engagement is vital as we collectively strive to enhance health equity and address Health-Related Social Needs (HRSN) across Oregon.
As a reminder, the application window for the 2025 Community Capacity Building Funding (CCBF) grant closes on May 30, 2025, at midnight. This grant represents an opportunity to increase organizational capacity to deliver non-medical Health-Related Social Needs services that improve health outcomes for Oregon Health Plan members.
In addition, we wanted to share some updates and reminders to ensure a smooth application process:
Updated Application Process: If you have previously visited Trillium's CCBF application, you will notice that some updates have been made. Instead of submitting applications via a web-based form, applications are now being accepted via a fillable PDF. Email your completed application and budget in their original file format to HRSN@TrilliumCHP.com. If you have already submitted your application, there is no need to resubmit. Updated instructions can be found on the 2025 CCBF website.
Additional Technical Assistance: To further support the application process, Trillium is offering an additional level of review for your Community Capacity Building Funding (CCBF) application. Applications received on or before Monday, May 19, will receive an initial review to ensure completeness and to identify any missing elements. Please note that if any items are missing and your application is submitted after May 19, Trillium cannot guarantee that these issues will be addressed, which may affect the consideration of your application. This additional review is designed to provide you with technical assistance to ensure your application has all the required elements.
OHA Clarification for Duplicate Applications: Trillium received clarification from the Oregon Health Authority (OHA) allowing CCBF applicants to submit duplicate applications to multiple coordinated care organizations (CCOs). This clarification allows for greater flexibility in the application process. If submitting duplicate applications to multiple CCOs, select “Yes” to question 17 and describe how funding is either duplicate or non-duplicate to multiple CCOs in question 18: "On 4/23/2025, Trillium received clarification from OHA regarding duplicate applications to multiple CCOs; the responsibility is on CCOs to assure non-duplicative funding.".
Required Budget Template: The budget template is a required component to your application and must be included—please do not modify the template from its original form. Completing it accurately will help us process your application efficiently and avoid any delays.
