Provider Listening Sessions: CCO Transition in Lane County
What’s happening: PacificSource Community Solutions (“PacificSource”) will end its role as a coordinated care organization (CCO) in Lane County in early 2026. This means Oregon Health Plan (OHP) members currently covered by PacificSource will move to Trillium Community Health Plan (“Trillium”). Oregon Health Authority (OHA), along with PacificSource and Trillium, is working to ensure a smooth process that supports both members and providers. Learn more about these changes.
About the listening sessions: OHA, Trillium and PacificSource invite affected Lane County providers to attend virtual listening sessions in December. These sessions will provide important information about what to expect, answer your questions, and gather your feedback to support a smooth transition for you and your patients.
If you cannot attend, you can share your questions and thoughts through our online survey until December 13. Your feedback will help OHA, PacificSource and Trillium provide the best possible support throughout this transition.
Provider Listening Sessions (Virtual Only):
- December 3, 7:00–8:00 a.m.
- December 4, noon–1:00 p.m.

Trillium 101: Access to Care Sessions

Please join us for “Trillium 101: Access to Care.” Facilitated by Kristinia Rogers and Lamar Tillman, Trillium’s Traditional Health Worker Liaisons, this interactive session for providers and community partners explores a variety of topics about our organization’s programs and services, including:

- Non-Emergent Medical Transportation (NEMT)

- Sessions take place on the fourth Tuesday of every month, January-June 2026, from 11:30 a.m.-12:30 p.m.

Health Net Oregon Clinical Policies Update
Effective December 1, 2025, Health Net of Oregon will be retiring the following Commercial clinical policies. You can find all active Commercial clinical policies on our Policies and Criteria webpage. (Note: After December 1, the below policies will no longer be posted on our website.)
Policy Retiring on December 1, 2025:
- Policy Name: CP.MP.167
- Policy Title: Intradiscal Steroid Injections for Pain Management
- Justification: The associated CPT Code is not on the current Pre-authorization list.
