Provider Listening Session: CCO Transition in Lane County
What’s happening: PacificSource Community Solutions (“PacificSource”) will end its role as a coordinated care organization (CCO) in Lane County in early 2026. This means Oregon Health Plan (OHP) members currently covered by PacificSource will move to Trillium Community Health Plan (“Trillium”). Oregon Health Authority (OHA), along with PacificSource and Trillium, is working to ensure a smooth process that supports both members and providers. Learn more about these changes.
About the listening session: OHA, Trillium and PacificSource invite affected Lane County providers to attend a virtual listening session on December 4. This session will provide important information about what to expect, answer your questions, and gather your feedback to support a smooth transition for you and your patients.
If you cannot attend, you can share your questions and thoughts through our online survey until December 13. Your feedback will help OHA, PacificSource and Trillium provide the best possible support throughout this transition.
