Health-Related Social Needs Services
Health-Related Social Needs (HRSN) services are non-medical items or services that will help improve your health.
Note: If you are a community-based organization or provider helping Trillium members to request HRSN services, please refer to the information below for guidance or review the closed-loop referrals section on our HRSN Providers webpage for information on how to refer Trillium members.
Climate-related supports could include an air conditioner to help you stay cool in the summer or a heater to help you stay warm in the winter. A mini fridge to keep medication cold is also a climate-related support. There are also other types of climate-related supports.
Climate-related supports are available for qualifying members beginning in March 2024.

Housing supports could include help paying rent or help paying for utilities like gas, electricity and water. This could also include changes to your home for health and safety, such as installation of ramps, grab bars and drawer pulls. There are also other types of housing supports.
Housing supports are available for qualifying members beginning in November 2024.

Climate-related supports and housing supports are available now if you qualify. Nutrition supports will be available in 2025.
To get HRSN services, the state of Oregon says that you must:
- Be an Oregon Health Plan (OHP) member,
- You can be a Trillium member, a member of another coordinated care organization (CCO), or a member of the Fee for Service (FFS) program.
- Be in at least one of these life transitions. Additional criteria also applies for each type of HRSN service.
- Release from incarceration in the past 12 months
- Discharge from an Institution for Mental Disease (IMD) in the past 12 months
- Current or past involvement in the Oregon child welfare system
- Transitioning from Medicaid-only to dual eligibility (Medicaid and Medicare) status within the next three months or has transitioned in the past nine months
- Being homeless or at risk of becoming homeless.
