My Health Pays Rewards
You can earn dollars through My Health Pays Rewards for completing wellness activities. Earning rewards is easy! All the My Health Pays (MHP) Rewards Value Added Benefits (VABs) must have a claim on file to process. Your healthy reward incentives will be mailed within 60 days of your claims being received.
Trillium Community Health Plan (Trillium) cannot process My Health Pays Rewards without a claim available. MHP payments may take up to 6-8 weeks after the claims are processed.
Qualification Requirement: Complete healthy activities like a yearly child wellness exam, immunizations, dental care, or other health services that support your health.
Log into your member account for your current My Health Pays Rewards balance.
Use your My Health Pays rewards to help pay for:
- Everyday items at Walmart
- Utility bills (light, water, gas)
- Childcare
- Rent

You will receive your My Health Pays Visa® Prepaid Card when you earn your first reward from Trillium. Each time you complete a qualifying healthy activity, we are notified and your reward dollars will be added to your existing card. It's that simple!

$25 Child Well-Care Visits
One per calendar year. Ages 3-21 years.
$30 Infant Well Visits 0-15 months
$5 per doctor visit, total of 6 per year
$20 Infant Well Visits 15-30 months
$10 per doctor visit, 2 per year.
$25 Notification of Pregnancy
One per pregnancy.
$20 First Prenatal Visit
Date of service must be within 42 days of enrollment or in first trimester. Once per pregnancy.
$20 Prenatal Visits
Reward requires three claims to earn $20. Limited to three rewards per pregnancy.
$20 Postpartum Care
One per pregnancy. 7-84 days after delivery.
$20 HPV Vaccine
Once per year, ages 9-13
$25 CM Engagement
One per pregnancy.
$25 Preventive Dental Visit
One per calendar year. Ages 1-14.
