Wraparound
Wraparound is a team-based model. A wraparound program brings together youth, family members, friends, teachers, counselors, and caseworkers. They create a plan to address your child’s needs.
A wraparound program is family-driven and youth-guided. Your wraparound team uses shared decision-making and builds on your family’s strengths. Each approach is individualized and informed by the family's culture.
Your wraparound team meets at least once per month.

Who is eligible for Wraparound?
- Youth under 21 years old, who are eligible for the Oregon Health Plan
- Be struggling in several areas such as home, school, or social relationships
- Experience or involvement with:
- Special education
- Child welfare
- Juvenile justice
- Mental health
- Substance use/Addictions
- Developmental disabilities
- Chronic or complex medical conditions
